Waiver

Acknowledgment of Risk

I, the undersigned participant (or parent/guardian, if applicable), acknowledge that boxing, fitness training, sparring, and all related physical activities offered by Champ’s Boxing and Fitness (“the Gym”) involve inherent risks. These risks include, but are not limited to: physical injury, bruises, sprains, fractures, concussions, illness (including exposure to communicable diseases), and in rare cases, death. I understand that such injuries may result from my own actions, the actions or negligence of others, or the condition of the facility or equipment.

I voluntarily choose to participate in these activities with full knowledge of these risks.

Assumption of Risk

I hereby knowingly and freely assume all risks, both known and unknown, even those arising from the negligence of the Gym, its owners, instructors, trainers, employees, or agents, and I take full responsibility for my participation.

Release of Liability

In consideration of being permitted to participate in any boxing, fitness, or training activity at Champ’s Boxing and Fitness, I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and hold harmless Champ’s Boxing and Fitness, its owners, employees, independent contractors, trainers, volunteers, agents, landlords, successors, and assigns (collectively “Released Parties”) from any and all claims, liabilities, demands, or causes of action related to any loss, damage, injury, or death that may occur while I am in, upon, or about the premises or while participating in any activity sponsored or organized by the Gym.

This release includes claims based on the ordinary negligence of the Released Parties.

Medical Fitness and Treatment

I certify that I am physically fit to participate in the activities at Champ’s Boxing and Fitness, and that I have no medical condition which would prevent my safe participation. I consent to receive emergency medical treatment that may be deemed advisable in the event of injury, accident, or illness during participation.

I understand that I am responsible for my own medical insurance and expenses.

Rules and Conduct

I agree to follow all Gym rules, instructions, and safety guidelines. Failure to comply may result in termination of membership or participation without refund.

Minor Participant (if applicable)

If the participant is under 18 years of age, this section must be signed by a parent or legal guardian:

I, the undersigned parent or guardian, consent to my child’s participation in activities at Champ’s Boxing and Fitness and agree to all terms set forth in this agreement on behalf of the minor. I further agree to indemnify and hold harmless the Released Parties from any claims related to my child’s participation.

Acknowledgment and Signature
